Saskatchewan! Healthy people. A healthy province.

All new Saskatchewan residents living in the province may complete and submit their application online.

The online application is a convenient and trouble-free way to apply for a Saskatchewan Health services card. The application guides you step by step to ensure all required information is entered to enable your application to be processed without delay.

The online application will asked for the name, address, and date of birth for all family members.

To complete your online application, electronic copies of supporting documentation are required. Electronic copies of your supporting documents can be obtained by scanning or taking digital pictures of your documents. Ensure that any numbers that appear on your documents are readable.

You may apply for Saskatchewan Health benefits for yourself, your spouse/partner and all dependents under 18 years old living in Saskatchewan. Dependants 18 years old and over must complete their own application.

Before you begin...please have your supporting documents ready.

Each adult must provide documents proving:

  • Legal Entitlement to be in Canada such as a Canadian birth certificate, Permanent Resident card, study permit with a student certification form or a work permit (An Open work permit requires a letter from your employer).
  • Saskatchewan Residency such as a utility bill, mortgage/rental/lease agreement, insurance policy or valid Saskatchewan vehicle registration.
  • Support of Identity such as your previous provincial health card, Saskatchewan driver's license or passport.

Each dependant must provide documents proving:
